Stabilisation and resuscitation of extremely preterm newborns at birth.

Stabilisation and resuscitation in the delivery room is an integral part of the care of extremely premature newborns. The main task is to support essential life functions and to facilitate the adaptation of the immature organism to the extrauterine life. The current recommendations are well defined for the full term and late preterm newborns, but there is a lack of targeted recommendations for the stabilisation and resuscitation of extremely premature newborns. The research part of the submitted thesis summarises the most important results of clinical research performed in 2010-2015 at the Department of Neonatology of the Department of Gynecology and Obstetrics, VFN and First Faculty of Medicine, Charles University in Prague. The research evaluates the efficacy and side effects of the currently recommended methods of stabilisation and resuscitation of extremely premature neonates in the delivery room and presents some new and potentially useful techniques for delivery room care. A significant output of this work is the recommendation for practice, structured according to the gestational age of extremely premature newborns. Nursing care about woman with diagnosis fetus mortuus in delivery room.

This bachelor thesis deals with the issue of dead fetus. The thesis has a theoretical and practical part. In the theoretical part, the terminology distinguishing birth and abortion, delivery of a dead fetus including the appropriate use of epidural analgesia are analyzed in individual chapters. I also deal with psychological issues that occur during childbirth. The last chapter of the theoretical part is devoted to social and legal support. The practical part is devoted to cases from the birth hall, where I analyze a particular case from the delivery room. The main emphasis is placed on the importance of the nursing process in the care of a woman with a suitable determination of nursing diagnoses and their complete realization. The output of my bachelor thesis is a recommendation for a practice devoted. The work as a whole is intended for midwives and midwifery students. The role of midwife at the birth management techniques Frederick Leboyer A birth is an event of a great importance not only for a woman but also for a child, who should be taken into consideration as well. According to Frederick Leboyer, it is important to welcome a newborn into life in the best way possible, as the birth has a major impact on the rest of the newborn´s life. Frederick Leboyer, a French gyneacologist and obstetrician, has made an assertion that birth trauma should be minimized and maximum ephasis should be laid on a newborn child. The theoretical part of my bachelor thesis deals with Frederick Leboyer´s birthing method and the role of a birth attendant performing a childbirth in accordance with the Leboyer´s method. The birth according to Leboyer is divided into an obstetric and a paediatric part. The obstetric part includes the psychological preparation of the mother, silence during the birth, patience, attention, deep concentration and birthing techniques. The paediatric part includes the observation of the beginning of the newborn´s breathing, a postponed cut of the umbilical cord, the newborn´s back massage and bathing of the newborn after giving birth. The purpose of the bachelor thesis research part was to discover whether any elements of the Leboyer´s method are incorporated in birthing at the present time and whether mothers-to-be, given the possibility, would be agreeable to give birth according to Leboyer´s method. Additionally, midwives were asked for their opinion about the method. The participatory action research was done during observing a number of birthings which took place at the Departments of Gynecology and Obstetrics of several chosen hospitals. In addition, semi-standardized interview with midwives, who work at the hospitals, and their clients, who were interviewed after giving birth, is included in the research. The aim of the research was to discover to which degree is Leboyer´s method incorporated in delivery rooms. On the basis of this aim a research question was set: ?How is the method used during birthing in delivery rooms?? It was discovered that, in the hospitals where the research was done, some of Leboyer´s techniques, such as to lay a newborn on the mother´s abdomen, a massage of the newborn and the newborn´s presence by his mother during the whole stay in the delivery room, are used. This bachelor thesis might serve as study material for those who study midwifery and as informational material about the technique for birth attendants and mothers-to-be in Czech hospitals. This bachelor thesis may as well serve as inspiration for creating a Leboyer-based norm for birthing practices.
Communication in the Birth Suite.

Given the importance of pregnancy and childbirth to a woman, communication is a key feature contributing to the successful development of and control over that anxious period. An appropriate and correct communication prevents confusion and misunderstanding between clients and healthcare professionals. Everyone needs to communicate, especially if they are at a healthcare facility and expect healthcare professionals to come up with words of explanation, encouragement and support. The theoretical section of the work deals with communication between a birth assistant and a birthing woman. The general section summarizes communication in the birth suite and lists factors affecting communication, such as pain and childbirth training. The main objectives of the work were to determine the effect of childbirth training on communication between the expectant mother and healthcare professionals during childbirth, to see whether birth assistants were able to appropriately react to conditions accompanying childbirth and to find out whether successful communication with women in labor depended on birth assistants{\crq} expertise. The objectives have been met thanks to the research conducted. Qualitative survey by means of interviews was employed in the research section of the bachelor thesis. Eight birth assistants were interviewed: four out of them had worked in the birth suite for more than ten years and the others for one year. In addition, eight mothers were interviewed the day after they had given birth. Four of them gave birth for the first time and four of them for the fourth time. The interviews were prepared so that I could answer the following research questions: 1. What do birth assistants think about the effect of childbirth training on communication with mothers during childbirth? Most of the birth assistants answered that question in the affirmative. Expectant mothers who have been trained before childbirth communicate better and have more information and skills. 2. What do expectant mothers think about communication in the birth suite? Expectant mothers think that communication in the birth suite is positive from the very first contact and especially emphasize healthcare professionals{\crq} readiness, patience, attentiveness, professionalism and human touch. 3. Do birth assistants have sufficient knowledge and experience of communicating in the birth suite? Birth assistants with ten and more years{\crq} work experience have enough knowledge and experience, whereas birth assistants with a year{\crq}s experience of the birth suite lack the expertise. This work has attempted to shed light on communication by birth assistants. It can serve secondary school and college students and birth assistants and doctors who can use it as a resource and make an assessment of their communication skills and abilities and it can be a source of information to help improve communication by birth assistants.
